Every effort is made to make your vacation time here with us at Buddy Dive Resort an enjoyable one. It is with this in mind that we are giving you the chance to download our diving liability forms ahead of your arrival date. If you like you can already complete some of the sections prior to checking in with us at the Dive Shop. Of course we will make sure that all the necessary forms are completed correctly before you go diving.

Every diver wishing to dive at Buddy Dive Resort MUST have a Dive Log Sheet.

Download Dive log sheet and complete sections:

• Group Name (if you have one)
• First Name, Last Name
• Certifying Agency, Card #
• Certification Level & Certification Date.

We will help complete any remaining section on the form.

Every diver MUST complete a Liability Release form. And if you are an Enriched Air Diver and you wish to dive Nitrox, you MUST also complete a Nitrox Liability Release form.
